It usually lasts about 40 weeks, starting from the first day of your last menstrual period. These 40 weeks are grouped into three segments called trimesters. The first trimester lasts about 13 weeks. During the first two weeks, you weren't actually pregnant because of the way your due date is calculated. But, your body is preparing for pregnancy. During the second week, an egg is released from your ovary, in a process called ovulation. Around the end of the second week, the baby is conceived in a process called conception or fertilization. This happens when a sperm from the father merges with the egg from the mother, usually in the fallopian tubes. During conception, genetic material from the mother and father combine to form a unique genetic code, instantly determining sex, hair color, eye color, and hundreds of other characteristics. This new single cell, called a zygote or embryo, is the beginning of a new human being. During the third week, the zygote divides to form a hollow ball of cells called a blastocyst. The cells of the blastocyst begin to arrange themselves into two groups. The inner group will develop into your baby, and the outer group will form tissues to nourish and protect it. During week four, the blastocyst moves into the uterus, where it hatches out of its outer layer. Once freed from this outer layer, the blastocyst can embed itself in the thickened lining of the wall of the uterus, in a process called implantation. Weeks 5 to 10 are referred to as the embryonic period. All of the major organs start to grow such as the brain, spinal cord, and heart. The heart begins to beat during this period. Structures called the placenta and umbilical cord begin providing life support for the embryo. They bring nutrients and oxygen from the mother to the embryo. Bones and muscles start to grow beneath thin, transparent skin. Toward the end of the embryonic period, the embryo starts to look more human. At the end of week 10, the embryo is now called a fetus and is about 1.25-inches long from head to rump. During the next few weeks, the arms and legs grow longer and start to move. Fingers and toes finish developing, and the baby's face becomes well-formed. By the end of the first trimester, your baby is almost three inches long. Your body experiences many changes during the first trimester. This is due to changing levels of chemicals called hormones circulating through your body. The most common first sign of pregnancy is that your menstrual periods have stopped. You may have other symptoms as well, such as nausea, often called morning sickness, that can occur at any time of day, tender, swollen breasts, mood swings, constipation, weight loss or weight gain, craving or disliking certain foods, and feeling more tired than usual. You may have only a few of these symptoms or none at all. If you have any questions about how your baby is developing, or concerns about how you're feeling, talk to your healthcare provider. This video is going to focus more on the technical side of things that I did once I came off the pill in order to conceive (the doctors I saw, the tests I had done, and the tracking/charting I did while TTC). I will speak more about the emotional side (like how we knew we were ready for a baby) in a future video! In Sept 2020, I decided to go off the pill. Coming off the pill at 29, I was ready to begin the process of getting my natural cycle back on track, and my thought at that time was that I wanted to wait until the following September (2021) to begin trying to conceive. As you all know, we got pregnant in April 2021, so this plan obviously took a turn and we started trying much earlier than expected. Immediately after I came off the pill, I set up an appointment with my doctor. I did some bloodwork to check for antibodies (to see if I was up-to-date on certain vaccines) and Michael and I also did genetic testing. Because it came back that I needed two different vaccine boosters (MMR and Varicella), I was told to NOT try to get pregnant during this time since they are live vaccines. Fast forward to October (one month later), and I decided that I was officially ready to start trying. I decided that it would be better to start trying then and get pregnant SOONER rather than start trying when we initially said and then have that timeline pushed back if we were to encounter any issues. However, there were some road blocks during this time as I still needed to get my period back! In addition, my health was worsening during this period - I was getting terrible migraines daily and found out that I had celiac disease! There were a few things that I did immediately after coming off the pill. The first was tracking. I began tracking everything about my ovulation and cycle. I took my basal body temperature every morning, used OTC ovulation predictor kits, used digital OPKs, and checked my daily cervical fluid. My temperatures seemed normal, but I would ovulate about every 40 days (instead of every 28 days). Although it was not normal length-wise, the fact that I WAS ovulating at all was a good sign. These long ovulation windows lasted until December, when I began to experience all these crazy health issues, inducing stress, and causing my period to disappear completely. I went back to my OBGYN and got a progesterone supplement to ‘induce’ my period. Around January 2021, I cut out gluten completely, my headaches went away and my period came back on its own. During this time I got the greenlight to begin trying! One of the biggest questions I got was which days I had sex to get pregnant, and honestly, it’s going to be different for everyone because we all ovulate on different days! When I began to hit that ovulation stage, we started having sex every day throughout my fertile window. Ultimately, all the planning and prepping paid off, and we got pregnant that month! After two weeks, my temperature remained elevated instead of dropping back down, which was a sign of pregnancy! I show a side-by-side comparison of a positive pregnancy test result vs a negative pregnancy test result. I also show a time lapse of the pregnancy test progress, and I explain the control vs test lines as well. For the demonstration, I used a pregnancy test called "Assured," which I bought at the Dollar Tree. (Yes, those Dollar Tree pregnancy tests actually work, and they accurately tested positive for both of my pregnancies!) Pregnancy tests like the one I used in this video detect a hormone called hCG (human chorionic gonadotropin). This hormone is released after the placenta attaches to the uterine wall. Sperm cells mix with other fluids to make semen, which spurts out of your penis when you ejaculate. Eggs live in ovaries, and the hormones that control your menstrual cycle cause eggs to mature every month. These hormones also make the lining of your uterus thick and spongy, to prepare for a possible pregnancy. Every month, one mature egg leaves your ovary — this is called ovulation. Intracytoplasmic sperm injection, or ICSI, is a form of in vitro fertilization in which fertilization occurs outside of the body. First, egg cells are harvested and transferred to a special media in a laboratory dish. Within a few hours, a single sperm is injected through a fine needle into the center of an egg cell to aid in the process of fertilization. If successful, the cell will divide and form the beginning stages of an embryo. If necessary, the DNA of a single cell from an embryo may be checked to ensure that various genetic disorders are not present. Typically, several egg cells are harvested and fertilized at the same time then inserted back into the uterus to increase the chances that one will implant and develop into a successful pregnancy. Intracytoplasmic sperm injection (ICSI) is an in vitro fertilization (IVF) procedure in which a single sperm is injected directly into an egg. Defective sperm function remains the single most important cause of human infertility. Although certain severe forms of male infertility have a genetic origin, others may be the result of environmental factors. During the past decade, ICSI has been applied increasingly around the world to alleviate problems of severe male infertility in human patients who either could not be assisted by conventional IVF procedures or could not be accepted for IVF because too few motile and morphologically normal sperm were present in the ejaculate of the male partner. Indications: This procedure is most commonly used to overcome male infertility problems, although it may also be used where eggs cannot easily be penetrated by sperm, and occasionally in addition to sperm donation. It can be used in teratozoospermia, because once the egg is fertilized, abnormal sperm morphology does not appear to influence blastocyst development or blastocyst morphology. Even with severe teratozoospermia, microscopy can still detect the few sperm cells that have a "normal" morphology, allowing for optimal success rate. Procedure: ICSI is generally performed following a transvaginal oocyte retrieval procedure to extract one to several oocytes from a woman. In ICSI IVF, the male partner or a donor needs to provide a sperm sample on the same day when your eggs are collected. The pictured oocyte has an extruded polar body at about 12 o'clock indicating its maturity. The polar body is positioned at the 12 or 6 o'clock position, to ensure that the inserted micropipette does not disrupt the spindle inside the egg. After the procedure, the oocyte will be placed into cell culture and checked on the following day for signs of fertilization. In contrast, in natural fertilization sperm compete and when the first sperm penetrates the oolemma, the oolemma hardens to block the entry of any other sperm. Concern has been raised that in ICSI this sperm selection process is bypassed and the sperm is selected by the embryologist without any specific testing. However, in 2006 the FDA cleared a device that allows embryologists to select mature sperm for ICSI based on sperm binding to hyaluronan, the main constituent of the gel layer (cumulus oophorus) surrounding the oocyte. The device provides microscopic droplets of hyaluronan hydrogel attached to culture dish. The embryologist places the prepared sperm on the microdot, selects and captures sperm that bind to the dot. Basic research on the maturation of sperm shows that hyaluronan-binding sperm are more mature and show fewer DNA strand breaks and significantly lower levels of aneuploidy than the sperm population from which they were selected. A brand name for one such sperm selection device is PICSI. A recent clinical trial showed a sharp reduction in miscarriage with embryos derived from PICSI sperm selection. 'Washed' or 'unwashed' sperm may be used in the process. Live birth rate are significantly higher with progesterone to assist implantation in ICSI cycles.
