Hormonal acne as the name suggests has a significant hormonal component (technically there is a hormonal drive with normal acne however this import is not the primary source of the problem). Hormonal acne can occur in one of two scenarios. Normal hormone levels on the background of a sensitive sebaceous or oil gland or rarely increase levels of certain hormones including testosterone, prolactin, 17 hydroxy progesterone and DHEASs. Hence it can be said that hormonal acne is an intracrine disorder we are by sebaceous glands are sensitive to normal hormones. How does hormonal acne present? Hormonal acne typically occurs in females over the age of 20 sometimes lingering on to the mid 40s. This type of acne is deep, often affect the chin and jaw line area, and fluctuates with the menstrual cycle. Acne is also modulated by food in not uncommonly stress. What is the significance of hormonal acne? This type of acne has one of the highest scarring rate due to the fact that lesions are deep and picking is not uncommon. Unlike normal teenage acne, hormonal acne has not got a head – it's more resistant to certain treatments such as exfoliation. How can we treat hormonal acne? The best solution is to see a dermatologist early. They can undertake investigations to exclude hormonal causes and will give you a choice of either an anti-hormone medication such spironolactone or cyperterone acetate. They also would consider the use of oral isotretinoin if your acne is recalcitrant and resistant to normal medication. What can you try at home? Rarey, hormonal acne can be treated with DIY treatments including the use of salicylic acid wash twice a day. You can trial retinol 1% at night, benzoyl peroxide between 3 to 5% should be used on spots. This may have a drying affect however BPO does not treat the fundamental cause of this acne. Pimple patches are invaluable in the treatment of hormonal acne is this patch will give feedback to the patients in order to stop picking. Do not underestimate the importance of an anti-acne diet as this can be marginally effective. What other options are there? Clinical strength BHA salicylic acid peels or retinoid peels can be effective. Phototherapy namely light treatment especially in the blue and red light spectrum can help some cases. BBL Forever Clear or vascular lasers may provide additional help in reducing lesions. Our suggestion is that you try these home remedies for a definitive period of 4 to 8 weeks. The most effective acne treatment depends on the type and severity. For my skin of color patients, and those who scar easily, I’m also more aggressive with treatment knowing they are prone to these harder to treat post acne marks! If your acne is mild, certainly trying drugstore benzoyl peroxide, adapalene, and salicylic acid products is a good start. Hi, I’m Dr. Chad Hartmann, a board-certified dermatologist with U.S. Dermatology Partners, and today I want to talk to you about chest and back acne. Acne and chest and back acne are predominantly due to genetic factors and I think it's important for people to understand this because commonly I’ll have a patient who comes into the office with acne and feels that the acne is their fault because they're not cleaning the area enough or I’ll have a parent who brings in their child and thinks the acne is due to the child not washing the area or because their hair is going over the area causing the acne and this is rarely the case and unfortunately this can lead to a delay in diagnosis and treatment and when treatment is delayed this can result in scarring which is permanent. Chest and back acne is treated similar to other forms of acne, however, sometimes chest and back acne tends to be a little bit more stubborn and not respond to treatment and it's also sometimes more difficult to treat simply because of where it's located. It’s hard to apply topical agents to these areas. How we treat acne is normally determined by the severity of acne, if the acne is mild often times we'll start with topical treatments such as a cleanser or a topical antibiotic or a topical retinoid. When the acne is more moderate then we'll normally need to use an oral agent or something by mouth. Typically we'll use antibiotics by mouth and in female patients, we can consider either an oral contraceptive pill or a hormone-blocking agent when it's appropriate. If the acne is more severe or in patients who don't respond to less aggressive treatments Accutane is normally a very effective treatment in these patients. Acne is very common during the teen years but it can affect you during adulthood as well. So you keep your hair and skin well lubricated your body depends on sebaceous glands which sit just under your skin. These glands secrete an oily substance called sebum which coats your skin and hair to prevent them from drying out. Sebum travels up hair follicles and out through your pores onto the surface of your skin. Your hair follicles routinely shed dead skin cells which sebum carries out of your body. When your body produces extra sebum and dead skin cells, they can stick together and clog your pores resulting in skin blemishes. Then bacteria that normally exist in small amounts on your skin can flourish in the sebum in the clogged pore leading to inflammation. Depending on where the clog is located and if you have inflammation, acne may appear as whiteheads which are clogged follicles closed off from the air, blackheads which are clogged follicles that turn a darker color when the clog is exposed to air, pustules commonly called pimples which are inflamed follicles clogged with puss, or cysts which are larger, painful, puss-filled lumps going deep under the skin. Hormonal changes, particularly a rise in testosterone, can lead to sebum overproduction which is why acne often occurs during the teen years. However it can occur at any age. Other factors contributing to the development of acne are bacteria, certain medications, and genetics. If you have a mild case of acne your doctor may recommend an over the counter lotion with one of several active ingredients. Benzoyl peroxide kills bacteria, dries excess oil, and removes dead skin cells clogging pores. Salicylic acid slows the loss of skin cells to prevent clogged pores. It may also break down whiteheads and blackheads. Alpha hydroxy acids such as lactic acid help remove dead skin cells, reduce inflammation, and stimulate the growth of new smoother skin. Sulfur removes dead skin cells and dries excess oil. These are strong chemicals that may irritate your skin. Follow the directions for use exactly. If over the counter products are not effective, your dermatologist may prescribe stronger prescription lotions such as vitamin A which reduces the buildup of dead skin cells in your pores, topical antibiotics which kill bacteria on your skin, or a combination of benzoyl peroxide and topical antibiotics. For moderate to severe cases your dermatologist may prescribe an oral medication alone or in combination with a topical treatment. These medications include oral antibiotics which kill bacteria and reduce inflammation and isotretinoin which is used only for the most severe cases. A corticosteroid injection may be given to relieve your pain and help clear up a particularly large lesion. For women birth control pills containing estrogen may be prescribed to minimize the effects of testosterone. Regardless of the treatment your doctor recommends, good skin care is essential. For example wash problem areas twice daily with a mild soap and wash gently without scrubbing. If you have dry or peeling skin use an oil-free water-based moisturizer. When choosing any product to put on your skin look for an oil-free or noncomedogenic label, which means it won't clog your pores. Avoid picking or squeezing blemishes as these actions may lead to infection or scarring. And avoid touching your face with your hands, your hair, or any object such as a cell phone. ANH12060

You can take curd, cheese and butter in moderation. ❌Food with High glycemic index such as sugar, chocolates, fruit juices, soft drinks,white bread, fast food, potato What to take instead of milk: ➡️Almond milk is an option ❌Avoid soy milk as that may exacerbate acne Frequently asked questions: ❓How to get calcium if no milk? ➡️You can increase consumption of non milk sources of calcium such as :Spinach, mustard greens, broccoli, lentils, chickpeas, flax seeds, chia seeds, sunflower seeds, nuts. ❓I don’t get acne but should I stop taking milk to avoid getting it? ➡️No, you can take milk albeit in moderation. ❓Even after stopping milk my acne is not getting better. What to do? ➡️Just stopping milk is not the solution. There are many other reasons which have to be addressed ❓I stay in a hostel. I don’t have access to healthy meals. In this weeks video I've first given a short explanation of why we get acne followed by tips on skincare, treatment and what can make acne worse. So if you have any friends or family who would benefit from this video then feel free to spread the word. Thank you. TYPE OF ACNE SPOTS: There are six main types of spot caused by acne, • Blackheads – small black bumps that develop on the skin; they're not filled with dirt, but are black because the inner lining of the hair follicle produces pigmentation. • Whiteheads – have a similar appearance to blackheads, but may be firmer and won't empty when squeezed. • Papules – small red bumps that may feel tender or sore. • Pustules – similar to papules, but have a white tip in the centre, caused by a build-up of pus. • Nodules – large hard lumps that build up beneath the surface of the skin and can be painful. • Cysts – the most severe type of spot caused by acne; they're large pus-filled lumps that look similar to boils and carry the greatest risk of causing permanent scarring WHAT CAN I DO IF I HAVE ACNE: These self-help techniques may be useful: • Don't wash affected areas of skin more than twice a day. Frequent washing can irritate the skin and make symptoms worse. • Wash the affected area with a mild soap or cleanser and lukewarm water. Very hot or cold water can make acne worse. • Don't try to "clean out" blackheads or squeeze spots. This can make them worse and cause permanent scarring. • Completely remove make-up before going to bed. • If dry skin is a problem, use a fragrance-free, water-based emollient. • Change your pillow case and face towel daily with a fresh laundered one. When not washed frequently, pillowcases and towels can contribute to acne breakouts because of the accumulation of bacteria, dirt, and oils. These come from the environment and from our own skin and hair from daily use. This small tip helped me a lot with my acne. Although acne can't be cured, it can be controlled with treatment that are available at pharmacies. WHEN TO SEEK MEDICAL ADVICE: Even mild cases of acne can cause distress. If your acne is making you feel very unhappy or you can't control your spots with over-the-counter medication, see your GP. Also see your GP if you develop nodules or cysts, as they need to be treated properly to avoid scarring. Try to resist the temptation to pick or squeeze the spots, as this can lead to permanent scarring. Treatments can take up to three months to work, so don't expect results overnight. HOW TO REDUCE SKIN IRRITATION OF BENZOYL PEROXIDE: If your skin does become irritated then stop using it until the irritation goes. Then try again with a lower strength, or reduce the time it is left on your skin before washing off. To prevent skin irritation, the following may help: • Most people can tolerate the 5% preparation but if it irritates then try the 2.5% once the irritation settles. If you wish to increase the strength, do it gradually. • Use a water-based preparation (rather than an alcohol-based one). • Apply once daily at first and wash off after several hours. • Gradually increase the length of time left on the skin. • Aim to put on twice daily when you get used to it. About prescription medication to fight acne, Natalie Suarez, Physician Assistant at Sunset Dermatology, says antibiotics are very frequently used for acne purposes, they help to reduce inflammation in the skin. "It's really not so much to kill bacteria, but that's something that you don't want to be taking long term because you can run into some issues with potential antibiotic resistance which can be a big problem," she explains. She recommends patients to be compliant with the instructions, because acne is a chronic condition and if you are not consistent with the treatment, it won't go away.

Before going into treatment, lets explore how to tell if you suffer from hormonal acne. 1. You are no longer a teenager. While we’d love to leave acne behind once we enter our 20s, the reality is flare-ups are possible at any time. Hormonal acne is the type that’s most likely to present in your 20-something years – all the way to your late20s. That because those are the years when women are most hormonally active. Your 20s is often peak childbearing age, making women more prone to the intense hormonal fluctuations conditions such as polycystic ovarian syndrome, childbirth, pregnancy and breastfeeding. Age alone won't determine whether or not you will suffer from adult acne. Genetics can dictate when hormonal breakouts start and stop. This is modified by diet, stress and the products you use. 2. One of the telltale signs of a hormonal breakout is its location on the face. If you’re noticing inflamed cysts around your lower face—especially your chin and jawline area—you can bet your bottom dollar that it’s probably hormonal. This is due to the fact that acne glands, known as sebaceous glands are concentrated in this area. 3. Period cycles and patterns. Adult hormonal acne often occurs in a cyclical pattern, much like women’s menstrual cycles. This is true even in postmenopausal women, because these women still experience monthly fluctuations in their estrogen and progesterone levels, albeit lower than pre-menopausal women. Adult hormonal acne tend to occur in the same place each month. 4. Stressors play a major role in adult acne. With our stressful jobs, cortisol or cortisone levels rise, this in turn stimulates the oil gland to enlarge and produce more sebum, clogging the oil gland to produce painful cysts. That is why taking a vacation resets our cortisol patterns and acne usually improves. 5. Deep painful cysts are the predominant lesion of adult acne, with very little in the way of zits, and blackheads. These painful cysts manifest as deeper bumps that are under the skin’s surface and cannot be extracted with normal manual extraction. How to treat? Here is the truth- this form of acne is one of the hardest DIY acne sub types to treat. Why? Because cysts lie deep in the dermal layers of your skin, beyond normal therapies such as creams, chemical peels and even laser light! Try these hints before seeing a dermatologist. Firstly review your skin care- use a simple cleanser, and non-comedogenic products. This goes across the board from your vitamin A, B, C creams and serums, sunscreen, moisturiser to your make up range. Mineral make up is considered the best for any form of acne, including hormonal acne. Secondly identify and rectify your flare up factors. Flare factors include foods such as dairy and refined foods high in sugars. Eat a balanced diet, and supplement this with zinc. Thirdly add vitamin A creams. In Australia buy a high strength retinol serum or cream and apply nightly to acne prone areas. In the US you can buy Differin. The trick is to constantly use this retinoid- its designed to prevent acne and not to be used as a spot treatment. If you after a natural solution- tea tree oil may help. The use of anti-inflammatory antibiotics may help some patients. Additionally the use of BHA or AHA peels and Blue/ Red light one week before breakouts can help some patients. Kleserca can be successful in some cases. So, how do medical dermatologist treat hormonal acne? Firstly they may investigate your condition and conduct a full hormone profile. Secondly they may treat you with the pill. OCP or oral contraceptive pills such as Diane, Estelle, Yaz or Yazaim may help. It often takes 3 period cycles to gain hormonal control. Medication such as spironolactone or cyperterone acetate are highly effective, and in my option are much safer than Accutane or Oratane. Oral retinoids are the last line of defense in the fight to control adult hormonal acne, they have many side effects and regular blood tests are required.
